Under the direction and support of the Assistant Team Manager, you will be working for the Children With Disabilities Team to coordinate and provide a service to children who are subject to child in need plans, child protection plans, pre-proceedings or court proceedings.
Provide day-to-day support, including activities, appointments, and family contact, in line with individual care plans.
