You will be working with a leading health and care charity who work with individuals, families that are affected by drugs, alcohol, crime, homelessness, domestic abuse and antisocial behaviour.
We are seeking an experienced Level 3 Social Worker to join our HICaF Discharge Team on a temporary basis for an initial period of 3 months.
The post holder will work closely within an integrated hospital discharge team environment to ensure that patients are discharged safely, with appropriate support in place, and to help alleviate the strain on hospital resources.
This role is critical in supporting the safe and timely facilitation of hospital discharges, especially as the Acute Trust faces increased pressures, particularly during the approaching winter months.
