Adult Social Worker - Level 3

We are seeking an experienced Level 3 Social Worker to join our HICaF Discharge Team on a temporary basis for an initial period of 3 months. This role is critical in supporting the safe and timely facilitation of hospital discharges, especially as the Acute Trust faces increased pressures, particularly during the approaching winter months. The post holder will work closely within an integrated hospital discharge team environment to ensure that patients are discharged safely, with appropriate support in place, and to help alleviate the strain on hospital resources.

**Key Responsibilities:

  • Facilitate safe and timely hospital discharges by working closely with the HICaF Discharge Team and other health and social care professionals.
  • Manage a complex caseload of adults, ensuring that assessments and care plans are completed in a timely manner to support discharge.
  • Conduct person-centred assessments to determine the needs of individuals and their families, ensuring that discharge plans are tailored to meet identified needs.
  • Collaborate with multidisciplinary teams, including nurses, doctors, therapists, and community partners, to develop discharge plans that support patients' safe transition from hospital to home or other appropriate settings.
  • Provide professional guidance and support to service users and their families during the discharge process, ensuring that they are fully informed and involved in decision-making.
  • Maintain accurate records and documentation using Eclipse (knowledge of Eclipse is desirable).
  • Adhere to relevant legislation, policies, and procedures to ensure compliance and best practice in all discharge planning activities.
  • Respond promptly and effectively to changing priorities and pressures within the team, demonstrating the ability to work under pressure.
  • Drive to various locations as required to support patients and facilitate discharges.

**Essential Criteria:

  • Qualified Social Worker with current registration with Social Work England.
  • Proven experience working within an Integrated Hospital Discharge Team, with the ability to hit the ground running.
  • Ability to manage a complex caseload and make sound decisions quickly and effectively.
  • Excellent communication, assessment, and organisational skills, with a commitment to collaborative working.
  • Knowledge of relevant legislation, including the Care Act, Mental Capacity Act, and Safeguarding Adults procedures.
  • Proficiency in using IT systems, including knowledge of Eclipse (desirable).
  • Full UK driving license and access to a car for work purposes.
