This rewarding role involves supporting individuals to maintain their independence, ensuring they receive tailored guidance and access to resources that enhance their quality of life. Key Responsibilities Provide advice and support to individuals, empowering them to live independently. Conduct assessments to identify support needs and develop action plans accordingly. Build strong relationships with individuals, families, and other professionals to coordinate and deliver effective support. Ensure the safety and wellbeing of service users by adhering to relevant policies and procedures. Maintain accurate and up-to-date records of interactions, assessments, and plans. Promote access to community resources, including housing, healthcare, and financial assistance. Conduct home visits and attend meetings as required. Requirements Full UK Driving License and access to a vehicle (essential). Experience in a similar role within a housing, social care, or community support setting. Excellent communication and interpersonal skills. Strong organizational and time-management abilities. Ability to work independently and adapt to a flexible schedule.
