About the Role

Key Responsibilities
- Provide advice and support to individuals, empowering them to live independently.
- Conduct assessments to identify support needs and develop action plans accordingly.
- Build strong relationships with individuals, families, and other professionals to coordinate and deliver effective support.
- Ensure the safety and wellbeing of service users by adhering to relevant policies and procedures.
- Maintain accurate and up-to-date records of interactions, assessments, and plans.
- Promote access to community resources, including housing, healthcare, and financial assistance.
- Conduct home visits and attend meetings as required.
Requirements
- Full UK Driving License and access to a vehicle (essential).
- Experience in a similar role within a housing, social care, or community support setting.
- Excellent communication and interpersonal skills.
- Strong organizational and time-management abilities.
- Ability to work independently and adapt to a flexible schedule.
