The work revolves around facilitating the transition from Children's Services to adult care, ensuring holistic planning and provision for their next stage in life.
We are looking for a passionate and professional Social Worker for a dedicated team within the 0-25 Service in Children's Services, focusing on supporting young people aged 16-25 with disabilities.
They operate under adult legislation, particularly The Care Act, and collaborate closely with Education colleagues and our Preparing for Adulthood Team, embodying the 3 Conversations approach in South Gloucestershire.
In this role, you will be responsible for supporting the learning and development of students in a KS3 / KS4 with additional needs, including SEMH (Social and Emotional Mental Health) and Challenging Behaviours.
The ideal candidate will be available to work Monday to Friday from 8:30am to 3:30pm, five days a week.
You will work closely with the classroom teacher and fellow TA's to ensure the students gain the support they need to succeed.
Role Status: 35 hours per week (12 month contract)
