Social Worker - 16-25 Disabilities, South Gloucestershire

We are looking for a passionate and professional Social Worker for a dedicated team within the 0-25 Service in Children's Services, focusing on supporting young people aged 16-25 with disabilities. The work revolves around facilitating the transition from Children's Services to adult care, ensuring holistic planning and provision for their next stage in life. They operate under adult legislation, particularly The Care Act, and collaborate closely with Education colleagues and our Preparing for Adulthood Team, embodying the 3 Conversations approach in South Gloucestershire.

What You'll Be Doing:

  • Managing a caseload of young people in transition or recently transitioned from Children's Services, employing a holistic approach to plan for their future, encompassing education, provision, budgeting, and potentially moving away from home for the first time.
  • Conducting Assessments under The Care Act, advocating for funding for care packages, and coordinating service reviews with a multi-agency perspective.
  • Assisting families in understanding the transition to adulthood for their young person, emphasising their ability to make independent choices and navigating the complex process with various professionals and legislative frameworks.
  • Participating in and managing our duty system, serving as the primary point of contact for Social Workers and Social Work Assistants, providing guidance on referrals and escalating complex decision-making situations when necessary.

What We Need From You:

  • Qualified social worker with current Social Work England registration, equipped with a solid understanding of asset and strengths-based approaches to social work.
  • Demonstrated specialist knowledge in Adult care and disability services, including proficiency in relevant legislation such as The Care Act, Mental Capacity Act, EHCP Code of Practice, Safeguarding Adults Policies and Procedures, and Deprivation of Liberty responsibilities.
  • Familiarity with the 3 Conversations methodology, which is integral to our service delivery in South Gloucestershire.
  • Proficiency in Mosaic or willingness to quickly familiarise yourself with the system.
  • Proven ability to assess, plan, provide, and review services for vulnerable young adults in the community, with a focus on risk management.
  • Sound understanding of funding transitions from Children's to adult services, including CHC considerations post the 18th birthday.
