What you'll be responsible for
Provide comprehensive secretarial and administrative support to the community clinics and associated health professionals and will actively participate and contribute to the development of the service.
The post holder will work closely within an integrated hospital discharge team environment to ensure that patients are discharged safely, with appropriate support in place, and to help alleviate the strain on hospital resources.
This role is critical in supporting the safe and timely facilitation of hospital discharges, especially as the Acute Trust faces increased pressures, particularly during the approaching winter months.
We are seeking an experienced Level 3 Social Worker to join our HICaF Discharge Team on a temporary basis for an initial period of 3 months.
The role will be initially on a 3 month fixed term basis with the potential to be extended for the right candidate.
Assist with the sourcing, monitoring and quality assuring placements for children and young people in the care of Halton Borough Council, which includes young people aged up to 18 and children with disabilities.
We are seeking an Placement Support Officer to work within Children's Services at Halton Borough Council.
