Responsibilities
Provide support and guidance to young people aged 14 and above as they prepare to leave care and transition to independence.
Within this role you will be working with children in the community.
You will be working minimum 2:1, ensuring the care and safety of a young service user between the age of 8-18.
For this role, you will need to have good attention to detail, writing detailed notes about the young service users' emotions, interactions with colleagues/family members, conversations in person and on the phone, food diaries and the daily routines.
