To undertake assessments of individual and carers needs and risks to encourage independence, provide support and prevent harm.
To undertake statutory intervention where service users are deemed to be at risk, including situations where people may be neglected or abused in accordance with the Inter-Agency Policy and Procedures for Safeguarding Adults.
This includes promoting a multi-agency approach by developing and maintaining effective working relationships.

Main Duties and Responsibilities
Act as an officer of the Local Authority and execute statutory functions to safeguard individuals and promote positive outcomes in accordance with legal requirements.
Tripod Partners is looking to place a full-time (37 hours per week) experienced social worker on the Mental Health Review Team based within integrated secondary mental health services.
There may also be an element of arranging services where there has been a change in need and also working with the psychiatrist/treatment team in the event the person were to relapse.
The primary function of the team is to undertake reassessments of need under the Care Act and s117, for people who have a diagnosed severe and enduring mental illness, who are under psychiatry and in receipt of a commissioned package of support.
