One position is within the Homefirst and Urgent Response Team. The second is in the Discharge Hub team who support onward discharge planning from the acute hospital or a discharge to assess bed. Both are busy and fast paced teams and would require a dynamic worker with experience supporting hospital discharge and admission avoidance. The team are based in the office 2 days a week but expected to complete urgent visits at short notice.
