This is an exciting opportunity to work with stroke survivors and their families to support them following stroke.
Services are contracted and there is currently funding for this contract until 31 March 2025.
You will be responsible for conducting assessments, creating, and reviewing support and safety plans to ensure high-quality support that promotes independent living.
This role involves developing client/family or children and young people-led support using person-centered approaches.
