A Local Authority in the West Midlands is looking to appoint a Quality of Placements Officer which will work as a link between the Local Authorities Placement service and those providers of Care that have been commissioned.
The role will focus on working with Care Providers to ensure that they are providing a service that i in line with the Local Authorities expectations whilst also ensuring Care Plans are followed.
Supporting the team to arrange Support for people in their own homes, on discharge from hospital or after a Care Act Assessment has taken place.
Liaising with Social Workers, Families, Providers and Quality Assurance to ensure the right care is delivered at the right time for the residents of Bromley.
The post-holder will be part of the Commissioning Team and work closely with Service Managers, Senior Practitioners and Care Managers to source, arrange and amend packages of care, placements in residential and nursing homes and supported living services.
Experience of hospital discharges and placements and knowledge of the Mosaic system would be an advantage.
This may involve regularly dealing with complex and/or contentious issues.
The postholder will be responsible to the Project Manager on specific matters relating to their projects.
£24 per hour umbrella; Contract (18 months)
