We have a new role available for a Housing Support Worker to join the charity to ensure that every client leaves a service with the skills and resilience to be confident and self-determining in their lives.
Care Navigator / Health and Wellbeing Support Workerwith experience in supporting vulnerable adults and excellent verbal and written communication skills, is required to join a well-established Charity.
You will be working with people in prison, and people on probation in the community.
KEY REQUIREMENT: Applicants must be 18 due to prison based working and hold a Full UK Driving Licence with access to own transport.

Health and Social Care Support Navigator with experience in supporting vulnerable adults and working knowledge of some of the following: health, debts, social care, housing, court systems, mental health, criminal justice and substance misuse, is required to a well-established Charity.
