Health and Social Care Support Navigator / Wellbeing Coach

This role is known internally as a Specialist Care Navigator

You will be working with people in prison, and people on probation in the community

SALARY: £25,147 - £27,596 per annum + Excellent Benefits (see below)

LOCATION: Covering the North East Region

JOB TYPE: Full-Time, Permanent

WORKING HOURS: 37.5 hours per week

KEY REQUIREMENT: Applicants must be 18+ due to prison based working and hold a Full UK Driving Licence with access to own transport.

The post will be subject to a DBS Enhanced with Adults barred check and Home Office Prison Clearance.

JOB OVERVIEW

We have a fantastic new job opportunity for a Health and Social Care Support Navigator with experience in supporting vulnerable adults and working knowledge of some of the following: health, debts, social care, housing, court systems, mental health, criminal justice and substance misuse.

As the Health and Social Care Support Navigator you will work with other health professionals, care navigators and health and wellbeing coaches to ensure holistic and meaningful support is provided to those with health vulnerabilities on their release from prison, ensuring continuity of care and fruition of release plans.

Working as the Health and Social Care Support Navigator you will use a coaching framework to manage wellbeing in the pre and post release period, to prevent a return to ill health as well as a reduction in reoffending.

By preventing this decline in health, through robust reconnection with health services in the community, will reduce health inequalities for those leaving the criminal justice system and provide support to allow a seamless, and effective transition to independent community living.

DUTIES

Your duties as the Health and Social Care Support Navigator will include:

  • Provide Specialist Health Care Navigation Services to individuals being released from the North East Prisons, supporting people both pre and post release in line with service guidelines, working alongside other agencies to improve health outcomes and reduce reoffending
  • Provide support to those being released from court, and provide assistance with these processes
  • Complete an initial screening and care mapping assessment with each service user and use service documents to plan their care and release
  • Develop tailored models of support for patients through a variety of modalities, mediums and with varying intensity to promote independent self-management and engagement with statutory and non-statutory services
  • Support health promotion, planning and delivering health promotion events in collaboration with colleagues
  • Complete, detailed, accurate, professional and timely notes for each patient
  • Provide a coaching framework to empower individuals with independent management of their release and associated healthcare needs

CANDIDATE REQUIREMENTS

  • Educated to NVQ Level 3 or equivalent with qualifications or training relevant to the role, e.g. Health and Social Care/Mental Health
  • Relevant experience of supporting vulnerable individuals to identify, set and meet their own goals using 1:1 and/or group interventions
  • Working knowledge of some of the following: health, debts, social care, housing, court systems, mental health, criminal justice and substance misuse
  • Experience of working to support individuals using structed, time limited support
  • Passionate and motivated to support individuals leaving the criminal justice system through provision of a high quality, professional level of care
  • Patient, calm and respectful
  • Hold a full UK driving license and have access to own transport for work purposes
