My client an established home care company that work with young adults in their own home is looking for a Care Coordinator to join their growing office-based team in Crawley.
This is a full-time position and previous experience in the care sector is essential, ideally as a Care Co-ordinator in a home care environment.
You should be based within a 15-mile radius of Crawley and have access to a car for work purposes.
The EHC Coordinator will act as the key point of contact for the child, young person and family, and will coordinate activities that are required to assess needs, meet specified outcomes and ensure the provision of holistic care and support.
The Coordinator will be the person who liaises with other professionals to ensure that the child/young person and their family have an organised and coordinated approach in order to meet their outcomes.
We are looking for and EHC Coordinator who will facilitate the coordination of roles across education, health and social care.
