Care Coordinator To provide safe and efficient service to service users, their families and local authority partners. The post holder will be responsible for the efficient planning of calls, resolution of issues and ongoing support for care staff and service users. The role will also include care work in the community. Must be a driver with own care. Must be able to commence on call every other weekend Duties & Responsibilities: To work as part of a team to maintain and update the data base with new service users, carers, cancelled and amended packages and extra calls. To produce weekly carer schedules and to review these with carers. To liaise with hospitals, next of kin and carers regarding hospital admissions and discharge. To record and update accurately essential details concerning both service users and carers. Ensure that the required information is present on carers rota's. To cover carer sickness and holiday. To hold regular area meetings and keep notes of each meeting and evidence how issues were resolved.
Care Coordinator Must be a driver with own care. Must be able to commence on call 1 in every 4 weeks! To coordinate the delivery of a safe and well planned domiciliary care service that is service user focussed. Duties & Responsibilities: To uphold the Organisation's vision & values and behaviour framework. To develop and maintain runs of work that are geographically planned, which use resources efficiently and continuously reviewed and updated as necessary. To produce staff rotas on a weekly/fortnightly basis, using the Organisation's computer based roster system. All records and rotas to be up to date at all times and be compliant to the organisational and legal requirements of a safe and well planned service. To allocate visits to care workers, taking into consideration service user's needs and preferences for gender and compatibility. To take responsibility for ensuring that all calls are covered in all instances, providing care personally if required.
