Care Coordinator

Reed Health and Care are recruiting on behalf of a leading home care provider in Southport, offering a range of services to support people to live independently and safely in their own homes. We are looking for a care coordinator to join our friendly and professional team and help us to provide the best possible care to our clients.

As a care co-ordinator, you will be responsible for:

  • Managing and co-ordinating the care packages of our clients, ensuring that they receive the right care at the right time from the right carer
  • Liaising with clients, families, carers, health and social care professionals and other agencies to ensure effective communication and collaboration
  • Monitoring and reviewing the quality and outcomes of the care delivered, and identifying and resolving any issues or concerns
  • Supervising care staff, and ensuring that they comply with the policies and procedures of the organisation and the relevant regulations and standards
  • Maintaining accurate and up-to-date records and reports using our electronic care management system
  • Participating in the on-call rota and providing support and guidance to care staff and clients as needed

To be successful in this role, you will need:

  • Preferred to have NVQ Level 3 in Health and Social Care or equivalent qualification
  • At least 1 years previous experience as a care coordinator or similar role in a home care setting
  • Excellent communication and interpersonal skills, both verbal and written
  • Ability to work under pressure and prioritise tasks
  • Ability to work flexibly and adapt to changing situations
  • Good IT skills and proficiency in using care management software
  • A full UK driving licence and access to a vehicle
