The Team Manager will undertake assessments, determine care packages/support plans and manage a range of the most complex caseload to maintain or improve the well-being of children and young people.
Plan, deliver, and oversee referral interventions for a complex caseload and where appropriate review cases.
Liaise internally and externally and work with colleagues from other agencies to ensure children and young people's interests are supported.
Ensure service users and their carers receive timely and proportionate care management assessments of their needs, that identify the outcomes they wish to achieve.
To work as part of multi agency teams providing seamless health and social care services.
Promote service user independence and recovery, including the promotion of Enablement and linking the service user to community based services that enhance personal wellbeing.
