We have a wonderful opportunity for a Case Manager to join our friendly Rehab Without Walls team to manage all the moving parts of the client's rehabilitation process, introducing the right therapists and services at the right time, coordinating and quality assuring services provided and maintaining effective working relationships with all the parties involved.

This is a remote role supporting clients across Birmingham and surrounding areas.
We are seeking a dedicated and experienced Case Manager to deliver the Drive Domestic Violence and Abuse (DVA) programme.
This role involves working one-on-one with perpetrators identified as high risk through MARAC and the Thames Valley Domestic Abuse Perpetrator Panel (DAPP).
We are looking for someone with strong experience of Case Management Software to take the lead on maintaining and developing their systems in line with business expectations across a number of offices in Kent.
You will need to have a strong understanding of design principles, system security as well as understanding cloud technologies and various frameworks, all ideally within a legal environment but this is not essential.
The post holder will act as a Case Manager for individual clients within Birmingham Solihull ICB who are receiving continuing health care funding to assess the needs of the client and their family to facilitate, plan and evaluate service provision to ensure the client achieves an optimum level of function and receives appropriate levels of care in the most suitable environment.
The post holder will undertake comprehensive professional Continuing Healthcare and Funded Nurse Care assessments for adults over 18 years of age with complex health needs including the assessment, review and monitoring of care for those whose health needs are funded via NHS Continuing Healthcare within the dedicated time scales.

You will conduct initial telephone consultations for our corporate clients and dependants who contact the Careline service and make appropriate recommendations for individualised wellbeing support.
