Overall Purpose of Job
To act as a member of staff for the Local Authority Adult Mental Health service providing an effective and efficient Care Act appropriate service to vulnerable adults with high mental health support needs and their carers, within the community or in a hospital setting.
This post is in the Mental Health Reablement Team, which is a short-term service that aims to prevent and reduce social care needs whilst promoting independence and recovery within the community.
Children's and Adults' Services
The aim is that, through a targeted, brief intervention, service users can enhance their wellbeing and independence and avoid longer-term social care support.

The role is to provide 1:1 and group support to young people with special educational, social and emotional difficulties.
