Location: North-East London but a community based role
This is a band 6 position in the home treatment and Reablement Integrated Independence Team
Start date: ASAP- 2month minimum
Shift pattern: full time, flexible shift pattern
Job description:
- Provide assessment and review for those following discharge from out of area hospitals returning home and those seen by Rapid Care to check functional ability and ensure safety at home including anticipatory oversight of need, scrutiny of discharge arrangements and promotion of single-handed care as part of the Discharge to Assess process
- Provide specialist occupational therapy input to the multidisciplinary screening of referrals
