Responsibilities:
- Provide essential support to older people in the community with mental health issues, physical and learning disabilities.
- Conduct Care Act assessments, develop and review care plans.
- Involvement in DSTs and court work where required.
- Manage a complex caseload while ensuring individuals receive the support they need.
To be considered for the role, ideally you will have:
- Minimum 2 years' experience in a similar social work setting.
- Diploma/Degree in Social Work.
- Registered with Social Work England.
- Proven experience handling complex cases, Care Act assessments, and DSTs.
