This is an opportunity to join the adults mental health hospital discharge team in Sheffield, with an hourly pay rate of £30ph, start date asap for 3 months minimum.
This is a hybrid role with 3 days in the office and 2 days from home.
You will need to be an experienced social worker with a mental health background.
Core tasks and duties as follows:
- Undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge
- Some mental capacity assessments, BI decisions and safeguarding as required
- Attending 117 meetings, ward rounds, professional meetings
- Facilitating learning opportunities for ASYEs through shadowing
- Participation in duty rota
- Successful candidate will be welcomed and integrated into the team and offered regular supervision and support from a 'hands-on' manager.
Minimum Requirements:
-SWE Registration
-2 year post qualifying experience within UK
-Eligibility to work in the UK
