Job Advert: Care & Support Coordinator

Myn is seeking a dedicated and experienced Care & Support Coordinator on behalf of our client. This role involves leading, supporting, and coordinating a team of Care and Support Workers to ensure high-quality care is provided to service users. You will work closely with service users and their families to develop personalised care plans and ensure all risk assessments are completed and up-to-date.

Key Responsibilities:

Develop and complete care and support plans and risk assessments tailored to individual service users' needs and abilities.

Supervise and support Care and Support Workers to ensure they deliver high standards of care in line with agreed plans.

Allocate tasks to Care and Support Workers, monitor their performance, and ensure they are equipped with the necessary knowledge and tools.

Conduct risk assessments, including Moving and Handling and environmental/lone worker assessments, ensuring safety for all.

Identify training needs and address them to ensure Care and Support Workers are continuously skilled and informed.

Ensure regular 1-to-1 and group supervision meetings are conducted in line with the organisation's policies.

Oversee the implementation of care plans aimed at promoting and maximising independence for service users.

Liaise with health and social care professionals to guarantee the effective delivery of care and support plans.

Regularly monitor and audit service user records, ensuring accuracy and relevance.

Address unsatisfactory performance and respond appropriately to emergency situations.

Assist with personal care needs and medication administration according to care plans, if necessary.

Uphold standards of dress and conduct for all Care and Support Workers.

Essential Skills & Experience:

Proven experience in a care supervisory or coordination role.

Strong knowledge of care planning and risk assessment processes.

Leadership and excellent communication skills.

Experience managing a team to ensure high standards of care delivery.

Understanding of relevant legislation and policies.
