Your new role
Southend-on-Sea City Council are looking for an Integration Lead on a full time and permanent basis. This contract is within an established team and is being offered on a hybrid work pattern (blend of working from home, out and about in the community and in the office).

As an Integration Lead, you will provide effective and visionary leadership across the health and social care system by utilising respective partner organisations / governance processes to drive effective integration between health and social care through collaboration with stakeholders and partners across the system to deliver better outcomes.

Working directly with the Head of Communities and Integration you will:

  • Provide effective system leadership and relationship management, building trust and developing shared priorities with partners and stakeholders
  • Assist commissioners to improve market shaping by addressing delayed transfers of care, admission avoidance, timely discharge and community-based solutions that promote independence and reduce the risk of social isolation.
  • Oversee and manage of the Better Care fund and other grant funds.
  • Deliver a program of activity to embed neighbourhood-based working within Southend.

What you'll need to succeed
To be successful in this role, it is essential that you are either educated to a degree level or have two years' equivalent experience in a similar role. It is also essential that you have excellent knowledge of current NHS (national, regional and local) priorities, local community as well as a knowledge and passion for co-production across Southend.

This role requires an individual with strong budget management skills and the ability to develop communities and community plans. This post is fundamental to driving positive change across the system within the city, so it is vital that you have sound knowledge of current national health and social care policy / system.To be successful in this role, previous experience of the leadership of a senior team working in a complex environment across health and/or social care as well as leading on the co-production and development of service is essential.
