Our client, an innovative local authority committed to recognizing potential, is seeking a high-performing Social Worker to join their Adults Social Work team. This is an exciting opportunity for a dedicated professional looking to advance their career and make a positive impact on young people transitioning to adult services.

  • *Key Responsibilities**:
  • Work as part of the Transition Service alongside the Children with Disabilities Team, Leaving Care Team, and SEN, providing support to young people transitioning to adult services.
  • Conduct timely and responsive assessments, re-assessments, and reviews, delivering personalized care to support users and carers while promoting maximum independence and safety.
  • Provide case management, EHCP's, health action planning, and person-centered planning to facilitate smooth transitions.
  • Participate in Continuing Health Care (CHC) Assessments and support young people into employment opportunities.
  • Liaise effectively and collaborate with statutory and voluntary organizations, as well as relevant agencies, to ensure comprehensive support for service users.
  • Contribute to team and service developments, representing the service as required, and sharing knowledge to enhance practice.
  • Undertake Mental Capacity Act assessments, Best Interest assessments, and Community Deprivation of Liberty Orders to safeguard vulnerable individuals.
  • Make referrals to supported living arrangements, initiate safeguarding enquiries, and develop risk assessments and protection plans as necessary.
  • *Requirements**:
  • Qualified Social Worker with relevant experience in adults' social work.
  • Knowledge of statutory duties under the Care Act and safeguarding procedures.
  • Strong assessment and case management skills, with the ability to deliver personalized care plans.
  • Experience working with young people transitioning to adult services is desirable.
  • Ability to collaborate effectively with multidisciplinary teams and external stakeholders.
  • Excellent communication and interpersonal skills.
  • Familiarity with relevant legislation, including the Mental Capacity Act.
