Role Overview

We are seeking a dedicated and compassionate Recovery Co-ordinator to manage a service user caseload, develop and review recovery plans, and work collaboratively with a multi-disciplinary team. The role involves close liaison with service users, their families, and various partner agencies to facilitate access to treatment and community resources.

Key Responsibilities

  • Manage a caseload of service users.
  • Devise, implement, and review ongoing recovery plans and treatment.
  • Engage with the full range of workers and skills within the service team, including family members and significant others as appropriate.
  • Conduct triage/screening assessments and comprehensive assessments with service users.
  • Work collaboratively with partner agencies for shared assessments, key working, and interventions.
  • Promote service user progression from engagement through structured treatment to self-sustained recovery.
  • Facilitate service user access to treatment and community resources to increase recovery and social capital.
  • Support service users and their families within their local community.
  • Lead effective case management, including risk assessments, risk management plans, and safeguarding assessments.
  • Utilize an online case management system efficiently.
  • Work closely with the multi-disciplinary team, providing reports and information.
  • Attend reviews, team meetings, and supervision sessions.
  • Communicate effectively via telephone and other means.

Qualifications and Skills

  • NVQ Level 3 in Health & Social Care (or equivalent) or willingness to work towards this qualification.
  • Excellent verbal and non-verbal communication skills to engage clients about their drug use while maintaining professional boundaries.
  • Strong ability to establish and maintain positive and productive working relationships within the team and with external partners.
  • Proficiency in MS Office programs, including Word and Outlook.
  • Ability to use a case management system effectively.
  • Understanding of the complex needs experienced by individuals with a history of problematic alcohol or drug misuse, including physical, psychological, mental health, legal, parenting, and family support needs.
  • Ability to assess complex needs, identify risks, formulate risk management plans, review recovery plans, and monitor outcomes in partnership with service users.
