Role: Occupational Therapist

Notes:
Community Therapist with ideally skills in crisis /hospital avoidance can be OT and Physio. Mainly for Taunton/west somerset/Bridgwater area
Rehab skills as needs to be able to identify goals and onward plans for patient.
Driver as need to drive across area and within 2 hour time frame for UCR visit.
Ability to provide manual handling assessment and knowledge of equipment is essential.
Flexible adaptable and able to manage /prioritise caseload load that change during the day.

- Location: Rapid response /UCR service - Taunton / Bridgwater area

We require someone who can work in the community to rapid response to patients needs- so flexible and adaptable no 2 days the same .
rapid/ucr provide urgent within 2 hours or same day response to patient who have gone into crisis in order to prevent hospital admission or quick turn around from And E department .
Need to be flexible and adaptable to work streams they are required in.
Car owner and driver essential.
Skills and knowledge within community working and or acute services. Will require a broad range of skills and experience within care of the elderly, medical and complex case management. Ability to work remotely and as part of a team and be flexible with case load working.
Work also involve eol work
Ideally looking for West side of somerset so Taunton/Bridgwater area covering of wider patch will be expected in time of service needs

- Grade: Band 6
- Location: Bridgwater Community Hospital wards
- Length: 2 months and then review monthly
- Hours: Full time preferred, could consider part time
- Preferred start date: ASAP
Notes:
Care of the older person
Rehabilitation in a non-acute inpatient setting
Discharge planning skills
MDT working
Clinical decision making
Will work as part of an established inpatient intermediate care team alongside PT, RA, pharmacy, nursing team, discharge facilitators & ASC.
Note that there is free onsite parking.
