Salary: £30,000 a year

Location: Stoke-on-Trent

Hours per week: 35

Contract type: Fixed Term Contract - 9 months

Our client is looking for someone to join and lead their Hospital Discharge Service located at the Royal Stoke Hospital. You'll provide a coordinated function within the department to support safe and timely discharges. Working with partners at the hospital, and within the voluntary and community sectors to support individual needs.

Job requirements

  • Line manage a small team.
  • Co-ordinate with the Manager and wider team for a seamless integration and communication process.
  • Collate quarterly monitoring data and attend project review meetings.
  • Lead and support colleagues by identifying patient resources.
  • A flexible approach to covering Co-ordinators role to ensure service demand is met.

  • Someone who's educated to a degree level or qualified with substantial experience.
  • Excellent communication skills.
  • An understanding of what great customer service is.
  • An ability to fulfil contractual commitments for commissioned services.
  • Ability to work under pressure and manage multiple tasks and demands.
  • Idea generation and implementation.
  • A motivational approach to help lead, coach, and support a team.
