We are currently seeking a dedicated and experienced Leaving Care Personal Advisor to join a supportive team based in Peterborough.

Job Summary:

As a Leaving Care Personal Advisor, you will be responsible for providing advice, support, and guidance to young people transitioning from care to independent living. You will work closely with young people to help them develop the skills and confidence needed to live independently and achieve their personal goals. This is a Hybrid role

Key Responsibilities:

  • Develop and maintain supportive relationships with young people leaving care.
  • Assess the needs of young people and create tailored pathway plans to support their transition to independence.
  • Provide practical support in areas such as housing, education, employment, health, and financial management.
  • Advocate on behalf of young people to ensure they receive the services and support they are entitled to.
  • Liaise with a range of professionals, including social workers, housing officers, and education providers, to coordinate care and support.
  • Offer emotional support and guidance to help young people cope with challenges and build resilience.
  • Monitor and review the progress of young people, adjusting support plans as necessary.
  • Maintain accurate and up-to-date records of interactions and interventions.
  • Ensure compliance with relevant legislation and policies related to leaving care.

Qualifications and Experience:

  • A relevant qualification in social work, youth work, or a related field.
  • Proven experience working with young people in a social care or support role.
  • Strong knowledge of the issues facing young people leaving care, including housing, education, and employment.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work independently and manage a caseload effectively.
  • Compassionate and empathetic approach to working with young people.
  • Proficiency in IT, including experience with relevant software and databases.
  • Must have own vehicle and a valid UK drivers license (desirable).
  • Must have relevant qualification NVQ 3 or equivalent.
