Job description:
- Assessment and treatment/discharge planning for patients admitted to older adult in-patients' sub-acute wards.
- Potential for being peripatetic across MHH, WHH, PTC and AMH.
- Experience of RIO desirable.
- Must have experience of Discharge to Assess process.
