Community Mental Health Nurse

One of our leading NHS clients is looking for experienced mental health nurses to cover shifts in the community hospitals in Manchester. We are seeking applications from Registered Mental Health Nurses who have experience working in community services.

Description:

This is a Band 6 community mental health nurse role working within Older Adult Services. This role involves responsibility for providing interventions to identified clients in a community setting, involving severe and enduring mental health issues and particularly challenging and complex cases.

Main duties will include:

  • Working in the community and liaising with in-patient areas, assessing and prioritising clients appropriately.
  • Managing a caseload of clients with complex and challenging mental health needs.
  • Assessing clients in the most appropriate location, often in their own homes, working in accordance with the Lone Working Policy.
  • Planning and implementing appropriate client-centered interventions to meet the needs of the individual.
  • Contributing to making decisions regarding client care in consultation with the multi-disciplinary team.
  • Maintaining standards regarding the quality of interventions provided.
  • Working within the All Agency Safeguarding Vulnerable Adults Policy framework and undertaking relevant training.
  • Carrying out a thorough holistic assessment of the service users' current mental health needs, including risk assessment and psychological needs.
  • Demonstrating the ability to make decisions in relation to the clinical management of individual patients and making critical clinical judgments in relation to mental health and care provision within the community setting.
  • Assessing and managing clients presenting with co-morbidities of physical and mental health needs, including the impact of medication.
  • Always maintaining confidentiality and a high professional standard.

  • Effective team working
  • Use of I.T. skills
  • Research and audit experience
