The main duties within this role are leading and supporting assessment, risk and care planning alongside CYP, their family/carers and the acute staff to safely facilitate discharge with follow up care planned in the community within 7 days. Where appropriate you will offer up to 6 follow up sessions with the CYP and ensure referral and signposting to appropriate services for longer term follow up is put in place and supportive discharge/transfer facilitated.

Clinical Responsibilities

You will work as an autonomous practitioner, undertaking clinical assessment of CYP who may have a complex co-morbid mental health needs and present in crisis, working alongside medical, nursing and other professional colleagues to ensure that the most effective care and treatment is provided resulting in safe, timely discharge from acute or T4 inpatient care.

To co-ordinate and liaise with Paediatric ward and SABP Core 13 (Paediatric Liaison Service) and the other CIS team members to facilitate timely discharge from hospital and follow up in the community.

Work closely with the Emergency Departments, the Adult Psychiatric Liaison Service, Mental Health Crisis Teams, On Call Duty Psychiatrist, On Call Managers, Core 13 (Paediatric Liaison Mental Health Nurses), ensuring CYP are seen in a timely manner and risk is assessed and a management plan is developed.

To manage own caseload from referral to discharge/transfer, holding medium to high risk cases and ensure timely and accurate documentation is completed.

Ensure that outcome measures for Crisis Intervention are implemented and audited. Contribute to relevant research and ensure that interventions on the pathway are evidence based.

Be responsible for the initial assessment and subsequent facilitation of referral to appropriate services once the crisis has become manageable.

Report to and advise community referrer on assessment and treatment outcomes and respond to requests for and provide formal reports for other agencies.

Be competent in conducting crisis assessments.
