Position: Young person's support worker
Hours: 37.5 per week
Shifts: Monday - Sunday, early (8AM - 4PM) and late (2:30PM - 10:30PM) shifts
Duration: Temporary, 2-6 months, possibility of extension
Location: Greenwich
We are seeking a dedicated Support worker for temporary cover. You will support service users by managing a caseload of 9 clients, conducting key working sessions, and developing Support Plans.
Key Responsibilities:
- Manage a caseload of 9 clients
- Conduct risk and needs assessments
- Respond to referrals promptly
- Identify care pathways and treatment options
- Build relationships with stakeholders and external agencies
- Help residents maximize benefits and manage finances
- Monitor and report maintenance issues
- Support clients with complex needs in a residential setting
