Job Description:
Are you an experienced social worker with a passion for hospital-based care and support?
In this role, you will be responsible for providing comprehensive assessments, planning, and interventions to support patients and their families.
Responsibilities:
- Conducting thorough assessments of patients' needs, risks, and strengths.
- Developing and implementing care plans to meet the individual needs and goals of patients.
- Collaborating with medical staff, families, and community resources to coordinate and improve patient care.
- Providing support and advocacy for patients and their families during hospital stays and transitions of care.
- Ensuring compliance with relevant legislation, policies, and procedures.
- Maintaining accurate and up-to-date records of assessments, interventions, and outcomes in line with legal and professional standards.
Qualifications:
- Qualified Social Worker registered with Social Work England.
- Proven experience in a social work role, preferably within a hospital or healthcare setting.
- Excellent communication and interpersonal skills, with the ability to build effective working relationships with patients, families, and medical staff.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Knowledge of relevant legislation, policies, and procedures related to social care and safeguarding.
