Our Client, Stockport Council, is seeking an Adults Social Worker to join their Hospital Team.

Responsibilities of this role:
- Working with Vulnerable Adults within a hospital setting
- Report writing and making assessments
- Developing care plans
- Care management
- Managing complex cases
- Liaising with other professional bodies
- Arranging safe and timely discharges for service users
- Self-directed support / personal budgets
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have at least 2 Years experience post qualified experience.
- Must have a minimum of 2 years experience working in a similar role.
- Must be able to work over a 7 day period.
