Job description:
- Providing rehabilitation alongside rapid response care. You will work particularly with frailty patients, those who are at risk of or who have fallen and for patients with long term conditions.
- You will need to have excellent communication skills, be able to demonstrate the ability to work in challenging environments and be committed to working autonomously, providing evidence based assessment, treatment and rehabilitation.
- A key focus is to provide care in patient's own homes to prevent hospital admissions, facilitate early discharge and where possible to enable patients to manage their own conditions and remain in their own homes, enhancing both their independence and quality of life.
