About the Team

We are a caring and committed team serving residents living in Woking who find themselves in need of drawing on care and support. We work with residents who are over 65, who may need care and support, as well as those 65 and younger who have physical disabilities, illness, or sensory impairments.

The Team works closely with carers, family members and partner organisations to enable residents to live their best possible life, without leaving anyone behind. We are committed to ensuring carers and family members feel supported, given the vital role they perform in the life of those who matter most to them.

We are a professional team made up of Social Care and Occupational Therapist Assistants and Social Workers and Occupational Therapists and are committed to learning and team development at both an individual and team level. Furthermore, we are always looking to grow in our ethnic diversity for our team to reflect the Woking community.

About the Role

As a Social Care Assistant in Surrey, you will work across various unique projects that ensure that the people we support can enjoy a home of their own, an environment in which they are supported to live a healthy & meaningful lifestyle.

Your role will be to assist qualified practitioners with a variety of tasks, including assessing people's needs and carrying out regular reviews. You will often be the first point of contact between the public and the service, gathering initial information and identifying the need for further assessment, as well as signposting people to other services.

You will work in partnership with individuals, their carers, professional staff in other agencies and service providers to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.

Social Care Assistants will undertake duty and hold a caseload of individuals and carers who will generally have a support plan in place that requires regular reviews.

You will have the opportunity to work on the new 'Discharge to Assess' model to support vulnerable residents leaving hospital settings in a timely and safe manner. This work will include working within hospitals as part of an integrated workforce of Health and Social Care practitioners with the aim of supporting our residents in their recovery in the community.

Shortlisting Criteria

To be considered for shortlisting for this position, your application will clearly evidence the following:

  • Understanding of Adult Social Care needs and any experience you may have in this area. It doesn't have to involve visiting people in their homes or in care settings
  • Your ability to manage a caseload efficiently, regularly monitoring support plans for individuals and carers
  • Experience of working directly with the public on the phone or in person
  • Your ability to build relationships with various stakeholders, colleagues, and the public effectively
  • Your IT skills with a willingness to learn to use a variety of software systems

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area.Reasonable adjustments where needed will be made for successful applicants who have a disability or long term health condition to enable them to fulfil the requirements of the job.
