Description:
The post holder will be responsible for assessing care needs and developing, implementing, and evaluating care programs for adults and older adults with mental health problems referred to the Crisis Resolution Home Treatment Team. Reporting daily to the Clinical Lead and accountable to the Matron for the overall service, the post holder will be a key member of a multi-disciplinary team, working closely with wards and Primary Health Care professionals. They will ensure care standards within the Crisis Resolution Home Treatment service and maintain an integrated service for clients while fostering a good interdisciplinary working environment.
Experience:
- Minimum of 2 years of experience working in the NHS, with at least 6 months in community settings.
Skills:
- Good understanding of crisis resolution home treatment service provision.
- Good understanding of positive risk-taking.
- Car driver / able to travel to all required locations and meet service activity requirements.
- Effective communication skills.
- Evidence of personal development.
- Ability to provide constructive feedback to staff.
- Ability to manage conflicts sensitively and decisively.
