South England Authority are looking for 4 Qualified Social Workers to work within an Adults Intermediate Care Team. Details of the role are:
The role is based within the Adults Intermediate Care Team, whose social workers undertake Care Act assessments and the first reviews, working across the whole of intermediate care including 2 inpatient units, the acute hospital and people in the community in their own homes and nursing and residential homes.

To apply for this role you must have:
- A relevant Social Work qualification (BA/MA Social Work/DipSW or equivalent)
- Be Social Work Registered
- Be eligible to work within the UK
