Role and Responsibilities:
- To support 0-6 assessment process
- To undertake service specific memory assessment tool (ACE III)
- Attend planned OPA and MDT discussion,
- To use trust record systems
Qualifications and Requirements:
- Nursing qualification
- Assessment experience
- UK driving licence and own vehicle
