Our Client, Calderdale Council, is seeking an Adults Team Leader to join their Hospital Discharge Team.

Responsibilities of this role:
- Ensure that effective systems are in place to screen referrals against eligibility criteria.
- To work with partners to ensure that referrals are dealt with by the creative deployment of resources both in terms of staffing and other options.
- Prioritise and allocate work to appropriate staff based on the complexity and urgency of a service request.
- To utilise the resources of the whole team and other skills within the overall service.
- Ensure the effective management and deployment of resources within budgets to meet agreed policy objectives and service priorities.
- To communicate effectively with the team about the effective management of budgets.
- Responsible for ensuring that the review and evaluation of Support Plans are carried out in accordance with practice guidelines, and that appropriate adjustments are made to Support Plans to reflect changing needs.
- Responsible for ensuring a systematic approach to the assessment planning, implementation and review of health needs in accordance with policies and procedures.
- Advise and consult with the appropriate Team Manager & Service Manager on expenditure for community care services which exceed current guidelines
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
- Must have minimum of 2 years experience working as a Qualified social worker in a similar team
