Community Mental Health Nurse

One of our leading NHS clients is looking for experienced mental health nurses to cover shifts in the community hospitals in Yorkside. We are seeking applications from Registered Mental Health Nurses with experience in community services.

Description:

This is a Band 6 community mental health nurse role within Older Adult Services. The role involves providing interventions to identified clients in a community setting, dealing with severe and enduring mental health issues, particularly challenging and complex cases.

Main duties:

  • Work in the community and liaise with in-patient areas to assess and prioritize clients appropriately.
  • Manage a caseload of clients with complex and challenging mental health needs.
  • Assess clients in the most appropriate location, often in their own homes, in accordance with the Lone Working Policy.
  • Plan and implement client-centered interventions to meet individual needs.
  • Contribute to decisions regarding client care in consultation with the multi-disciplinary team.
  • Maintain standards regarding the quality of interventions provided.
  • Work within the All Agency Safeguarding Vulnerable Adults Policy framework and undertake relevant training.
  • Conduct a thorough holistic assessment of service users' current mental health needs, including risk assessment and psychological needs.
  • Demonstrate the ability to make decisions regarding the clinical management of individual patients and make critical clinical judgments in relation to mental health and care provision within the community setting.
  • Assess and manage clients presenting with co-morbidities of physical and mental health needs, including the impact of medication.
  • Always maintain confidentiality and a high professional standard.

Requirements:

  • Experience in mental health
  • Relevant qualifications
  • Experience in community mental health
  • Broad experience within a service-related setting
  • Demonstrated achievements in developing care standards/procedures/guidelines
  • Effective team-working skills
  • Proficiency in IT
  • Experience in research and audit
  • Clinical audit experience
