Support Worker

Position: Wellbeing Practitioner
Type: Temporary (8 weeks), Part-time and Full-time vacancies available
Location: Various locations around Bury
Shift Patterns:

  • Full-time Split Shifts: 8:00 AM - 1:00 PM and 4:30 PM - 9:30 PM (10 hours per shift), working a 2-week rolling rota (including weekends/bank holidays)

  • Week 1: Monday, Tuesday, Saturday, and Sunday

  • Week 2: Wednesday, Thursday, and Friday

  • Part-time Evening Shifts: 4:30 PM - 9:30 PM (5 hours per shift), working a 2-week rolling rota (including weekends/bank holidays)

  • Week 1: Monday, Tuesday, Saturday, and Sunday

  • Week 2: Wednesday, Thursday, and Friday

Requirements:

  • Must be a driver and have access to a vehicle
  • Clear enhanced DBS check
  • Relevant experience in support work, moving and handling, etc.
  • Ability to work both independently and as part of a team

Responsibilities:

  1. Support and Reablement:
  • Promote the reablement of customers with identified needs.

  • Enable customers to reach their potential, maintain independence, and continue living in their own homes or return after a period of support.

  1. Person-Centred Care:
  • Provide therapeutic and creative solutions to assist recovery and encourage independence.

  • Reduce dependence on services through self-management.

  1. Goal Planning:
  • Contribute to the delivery, evaluation, and adjustment of goal plans to maximize independence outcomes.

  • Meet identified goals within agreed timelines.

  1. Daily Living Assistance:
  • Assist customers, including those with complex needs, with daily living tasks such as personal care.

  • Enable customers to undertake these tasks themselves in line with their personal goal plan and risk assessments.

  1. Assessment and Review:
  • Positively contribute to the assessment and review of customers' needs.

  • Support the creation of accurate and therapeutic goal plans.

  1. Communication:
  • Receive, transmit, and store information using appropriate methods (face-to-face, telephone, electronic).

  • Be aware of and react to any changes in customers' needs or service provision at the beginning of each visit.

  1. Customer Care:
  • Deliver outstanding personalized customer care.

  • Liaise with families, carers, and advocates professionally and courteously.

  • Respect confidential information in line with the authority's policies.

  1. Compliance:
  • Adhere to the appropriate policies, procedures, and values of the authority and department.

Tasks Include:

  • Supporting customers with washing/dressing, showering, meal preparation, and medication.
