Community Wellbeing Worker

Salary: £13.19 per hour

Our client is looking for someone to join their trusted team, working as part of their Wellbeing Support Service. As a Support Worker, you'll work closely with Adult Social Care to support customers being discharged home from hospital and those who need support to stay safe and well in their community.

Job requirements

Complete tasks to help customers return safely home from hospital. Including completing property inspections, collecting belongings, dropping off food parcels, and arranging quotes for deep cleans so discharge can go ahead.

Complete holistic assessments with customers using an action plan to ensure customers remain living independently, safe, and well in their home.

Complete ad-hoc and regular visits to people identified in the community as needing further support and complete tasks to support them as requested like, checking homes are warm and safe, checking medication, ensuring food and other essential items are available.

What they're looking for:

  • Excellent communication skills.
  • An aptitude for working with vulnerable and/or older people.
  • Understanding of what great customer service is.
  • Ability to work under pressure and manage your daily tasks.
  • Excellent ICT skills.
  • Excellent administrative skills.
