Job title: Social Worker, Adults, Central Doncaster

Job Type: Full time, Permanent

Hours: 37 hours per week

Salary: £31,372 - £41,438

Purpose:

Work directly with individuals and families to help them make changes and problem solve. Make a positive difference in people's lives. Listen and work with individuals to feel more empowered and in control of their own life. Also provide the social care support within the Neurological Rehabilitation Outreach Team (NROT), which offers specialist rehabilitation services for people with neurological conditions.

Responsibilities and Duties:

Conduct assessments of the needs and risks of adults in the community, using a strength-based approach.

Develop and review personalised care and support plans with the involvement of the person, their family and other relevant professionals.

Provide information, advice and guidance to people and their families on the available services and resources in their local area.

Liaise and collaborate with other agencies, such as health, housing, education, employment and voluntary sector, to ensure coordinated and holistic support for the person.

Safeguard the rights and wellbeing of adults at risk of abuse or neglect, and to report any concerns to the appropriate authorities.

Maintain accurate and up-to-date records of the work done with the person, using the relevant systems and tools.

Participate in regular supervision, training and team meetings, and to contribute to the continuous improvement of the service.

Adhere to the professional standards, codes of practice and ethical principles of social work.

Knowledge & Skills:

Knowledge of relevant legislation, policies and procedures

Excellent communication and interpersonal skills

A commitment to person-centred and strength-based practice

A flexible and adaptable approach to work

A willingness to learn and develop your skills

Experience:

Relevant post-qualification experience in adults social work within a statutory, voluntary, or independent sector organisation.

Qualifications:

CQSW / DIPSW / Degree in Social Work or equivalent.

Must be a driver with own car
