Falls & Frailty Programme Manager

Reed Public Sector is actively recruiting for a interim Programme Manager in Falls and Frailty for a Local Authority in Berkshire.

The purpose of this role is to undertake a diagnostic review of falls and frailty data across the council gathering information and data, to understand and describe existing pathways for falls services and identify gaps in services including preventative approaches to avoid hospital admissions due to falls.

You will be actively involved and assume a leadership role in Phase 1 of the falls and frailty project. This involves conducting a diagnostic review of falls throughout Berkshire West and subsequently developing a proposal for a falls and frailty pathway and service.

This role is well-suited for a highly organized individual with expertise and insight into Falls and Frailty within Adult Social Care and Health and Local Authorities.

We seek someone with a passion for making a positive impact, exceptional interpersonal skills, analytical thinking, and the ability to influence and instigate meaningful change.

Utilize population health data and collaborative strategic needs assessments to identify population groups vulnerable to avoidable admissions.

Conduct an analysis encompassing demographic trends, the influence of health and broader disparities, and the repercussions of local deprivation levels.

Working with internal and external partners, agree a system-wide interpretation of the combined data and insight so there is common understanding of local trends and each partner can model risk as they address the common agreed actions.
