Job Description:
We are seeking a compassionate and dedicated Polish / Romanian / Punjabi speaking Family Support Workers to join our team and work for a Care home based in Northolt. The successful candidate will work directly with families, providing support, guidance, and resources to help them overcome challenges and achieve stability. This role requires a deep understanding of family dynamics, child development, and community resources.
Key Responsibilities:
- Provide one-on-one support and case management to families in need.
- Conduct home visits to assess family needs and monitor progress.
- Develop and implement individualized family support plans.
- Connect families with appropriate community resources and services.
- Offer parenting education and support.
- Facilitate group sessions and workshops for parents and children.
- Advocate for families within the community and with service providers.
- Maintain accurate and confidential case records.
- Collaborate with other professionals and agencies to coordinate care.
Qualifications:
- Previous experience in family support, social work, or a similar role.
- Strong understanding of child development and family dynamics.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Ability to handle sensitive and confidential information with discretion.
- Flexibility to work some evenings and weekends as needed.
