Reablement Enabler

Seven Resourcing are looking for a Reablement Enabler to work with Islington council. This is a Part-time position, and we are paying £17.38Paye and £22.76 Umbrella per hour for this role.

The role:

The post holder will be part of the Islington Reablement Service – In-house provider services.

Reablement enables individuals to regain or maximise their independence through intensive, short-term support, building on their strengths and enabling people to live healthy independent lives for as long as possible in their own homes, or the place they call home. The Islington Reablement Service is for adults with a range of needs including learning disabilities, physical and sensory needs, mental health and older people both in community and hospital settings across various locations.

The service is currently delivered 365 days from 8am – 10pm. However, the future direction is for the services to become increasingly multi agency and the service may need to change in line with service development requirements.

Key Responsibilities:

To ensure that all there is a strengths-based approach demonstrated in the delivery of care and support as detailed in the persons Reablement plan

To carry out Reablement plans and provision of care and support

To report any safeguarding concerns immediately, whether confirmed or suspected, to the Duty Manager in line with the Councils Safeguarding Policies & Procedures

To ensure that the Care Act 2014 is implemented in all aspects of their role

To work with people using our services, carers, families, and other professionals to minimise and manage risk whilst enabling the service user to maintain their chosen lifestyle as far as possible

To provide support with medication as detailed in each person's Support Plan and in accordance with NICE Guidelines (NG67) and The Reablement Service Medication Policy. The term ‘medicines support' defined as any support that enables a person to manage their medicines**.**

Applicant Requirements:

  • Qualifications: GCSEs A-C or equivalent including at least English and Maths.

Educated to A Level standard or equivalent experience.

  • Experience: 2+ years working within a similar role.
  • Travel: Clean UK driving license with the ability to commute to and from work independently.
  • Hours: 36 hours p/w.
